The Disability Aid Trust (DAT) help towards the extra cost of helpers to ensure that severely disabled people can go on holiday or have a short break.
Applications are open to individuals and groups of disabled people and must be supported by a bona fide organisation, a doctor or social worker.
Funds are allocated twice a year:

Holidays with Help organises holidays for groups of disabled people.
The holidays are open to individuals, couples and family groups as well group bookings. The cost of the holiday includes accomodation, meals, activities and trained helpers.
They include a mix of indoor and outdoor activities with evening entertainment.
Care is provided, though you are welcome to bring your own carer. Medical and nursing advice is also available.

The Jumbulance Trust is a small national charity that enables severely disabled and unwell adults, children and dependent elderly persons to travel on holiday in the UK and Europe.
The Trust provides the vehicles and drivers at subsidised rates to groups around the UK and Ireland.
They have two large modern "Jumbulances" (coach/ambulances) that make it possible for those that need extra care or specialised facilities to travel.
Groups can hire a Jumbulance to take a party to see local places of interest or disabled supporters of their favourite sports club to watch a match or sporting event.
Jumbulances can also be hired for longer periods such as taking a group on holiday, to go skiing or on a pilgrimage to places all over mainland Europe, as far as Poland, Austria and Italy.
The Trust takes volunteers and a professional doctor on all of their trips to care for their travellers, giving their families a chance to relax without care responsibilities.

Yellow Submarine provides fully supported, subsidised residential holidays for adults (18+ years) with learning disabilities to destinations all across the UK and France. Applicants must live in Oxfordshire.
Yellow Submarine also provides a variety of projects for young people (11 to 18 years) with additional needs across Oxfordshire. This includes residential holidays, youth club and activity days.
The Home Farm Trust produce an annual holiday guide packed with information for any one with a learning disability and those closest to them.
